As a social worker, I am not an essential worker therefore I am not allowed to visit your loved one in a residential care facility or skilled nursing facility. Only essential workers such as licensed nurses are allowed to visit facilities at this time. I am also not qualified to provide instructions on CDC regulated PPE.
I am available to lower the risk of emergency room visits or hospitalization by offering families information on safety, prevention, and emergency preparation.
Some patients qualify for a Licensed Home Health Care order by their primary physician for a licensed nurse to come to the home for prevention of risk of emergency room visits or hospitalization. Licensed Home Health Care is covered 100 percent by Medicare. The patient must have a qualifying event which is a significant change in status. The patient has to be considered “homebound” in order to qualify for Licensed Home Health Care services. Services can include Licensed RN, Physical Therapist, Occupational Therapist and Home Health Care Aid depending on the needs of the patient.
Change in Status examples include:
A fall or increased risk of falls that requires a safety evaluation from a licensed Home Health Care physical therapist
A medication reaction that creates a major change in baseline status
A significant change in mental state or medical condition
Problems addressing areas such as high or low blood sugar, high or low blood pressure, any other medical changes that require you to be homebound and receiving treatment from Licensed Home Care provider
It is always worth a call to your physician for any change of status to request home care services and very often the order will go through.

(HHCI) Home health Care Inc. (click here) is locally owned and I have worked with them for over 20 years and they provide excellent services at the most competitive rates on the Bay Area. HHCI is a private Licensed Home Care agency that your family can hire to provide nursing supervision as well as caregiver services in your home. The patient may need a nursing evaluation to educate caregivers and family members to avoid risky emergency room visits or hospitalizations during COVID. The nurse can contact the patient’s physician and assist with communication and changes in medications or other updates on the care plan in the home for increased safety, comfort and prevention. During these times of infection rates rising in health care settings, it is important to take preventive measures and have emergency and change of status preparedness in mind at all times for vulnerable patients requiring care in their homes.
